An exciting opportunity has arisen to join the forward-thinking and dynamic PCN community team in Crowborough, Ashdown Forest, and Wadhurst catchment area.

This pivotal role will provide prompt, efficient co-ordination and administration support to PCN teams, playing a significant part in the day-to-day administration of the work in relation to Community Nursing.

Main duties of the job

  • This role provides prompt, efficient co-ordination and administration support to community nursing services.
  • You will be the main point of contact for service users, their families, other teams and organizations.
  • Promptly respond to incoming referrals from various sources, including : SystmOne, email and telephone.
  • The role will require effective communication skills via various methods with the ability to adapt this to support service users with complex needs.
  • You will be required to co-ordinate interventions by prioritising and allocating Care Plans to appropriate team members referring to Standard Operating Procedures and escalating where appropriate to the Senior Nurse on Duty.
  • The aim is to support the clinical team to deliver high standards of care by ensuring continuity of care and co-ordinating the service.

  • GCSE passes at Grade C or above or equivalent experience
  • Evidence of consistent pattern of learning from education, training and experience
  • Understanding of basic human needs - physical, emotional, health and social

Desirable

  • NVQ level 3 or equivalent (health and social care or business administration)
  • IT/Administration Qualification e.g. ECDL
  • Knowledge of assessments & Care
  • Management Process
  • Knowledge of medical terminology
  • Knowledge of social care & health systems, processes and pathways

Experience

Essential

  • Significant experience of working in a health or social care setting
  • Previous administration experience
  • Working with multi-disciplinary teams
  • Experience of working with vulnerable people

Desirable

  • Considerable experience needed in working with the public

Skills & Knowledge

  • Use of recognised IT packages e.g. Microsoft Office, Excel and Access
  • Effective keyboard/IT skills, which include accurate recording skills
