We are looking for a dynamic, enthusiastic and motivated individual to join our workforce in a busy and well established community team. Greater Wealden PCN provides nursing services to Heathfield, Uckfield and surrounding area. Our community nurses benefit from having continual opportunities to develop an excellent skillset and become autonomous practitioners whilst working as part of a team delivering high quality, holistic care to patients in their own homes.
Primary Care Networks (PCNs) are an innovative service development involving community nursing. The ambition is to integrate multidisciplinary professionals so health and social care can be delivered seamlessly to those who need it. Due to this, community nursing offers a dynamic nursing environment which enables nurses to become confident, skilled practitioners.
Main duties of the job
The main roles of the job:
- Holistic assessment and care planning for patients in their own homes..
- Individualised treatment and the promotion of self-care.
- Admission avoidance and enabling patients to remain at home where possible
- Cross service collaboration and referral to appropriate members of the multidisciplinary team.
- Development of extensive nursing skills with excellent training opportunities
- Professional development as required to maintain area of practice, asnegotiated in supervision.
- An awareness of health, safety and security risks
- Engagement in service provision and change.
- Acts as a practice supervisor for staff and students.
