Job summary
This is a fantastic opportunity for a passionate nurse who has a special interest in the field of HIV. If successful in this role you will be responsible for delivering high quality care to complex HIV patients in the community in West Sussex based in either Brighton or Crawley. You will be expected to work both as a team and autonomously. Your role will include holistic assessment, implementation, care planning and evaluation of care. You will be responsible for effective discharge planning and monitoring patients in the community, promoting a self-care model.
You will participate in the continual development of the Community HIV Specialist Service by ensuring the delivery of evidence-based care and sharing current practices between the professional team members and the wider MDT.
You will provide advice and support to patients, primary health care teams, and otherstatutory and voluntary agencies, and contribute to the development of clinicalpractice in respect of HIV treatment and care.
There may be times you may be required to cover the other teams in the CommunityHIV Specialist Service to ensure continuity of care and effective team working. Therole will involve travelling over a wide geographical area in West Sussex so it isessential that you have a car and are able to drive.
Main duties of the job
- Link with hospice, acute care and continuing care services, and develop links with statutory and voluntary care agencies in the assessment of care needs, care planning and service provision.
- To offer support and advice of all aspects of HIV and sexual health.
- To facilitate discharge planning from acute care and respite care.
- Work closely with primary health care teams in the planning and provision of care at home.
- Arrange joint visits with other appropriate practitioners to patients in hospital to facilitate transfer of care on discharge and to monitor and audit referrals.
- Participate in audit initiatives.
- Facilitate effective working relationships with all disciplines and agencies concerned with the care of people with HIV.
- Develop the role to address the issues of stigma and discrimination.
- Offer support and advice on health care issues to workers involved with HIV positive clients.
- To be available to give specialist nursing care to patients in the community (for example, health assessments and blood sampling),working closely with primary health care teams.
- Liaise with specialist acute services (for example, the dedicated HIV inpatient unit) in the development of care packages for support and in conjunction with the primary health care team.
