An exciting opportunity has arisen to join the forward-thinking and dynamic PCN community team in Rural North Chichester, based in Pulborough.
This pivotal role will provide prompt, efficient co-ordination and administration support to PCN teams, playing a significant part in the day-to-day administration of the work in relation to Community Nursing.
Main duties of the job
- This role provides prompt, efficient co-ordination and administration support to community nursing services.
- You will be the main point of contact for service users, their families, other teams and organizations.
- Promptly respond to incoming referrals from various sources, including : SystmOne, email and telephone.
- The role will require effective communication skills via various methods with the ability to adapt this to support service users with complex needs.
- You will be required to co-ordinate interventions by prioritising and allocating Care Plans to appropriate team members referring to Standard Operating Procedures and escalating where appropriate to the Senior Nurse on Duty.
- The aim is to support the clinical team to deliver high standards of care by ensuring continuity of care and co-ordinating the service.
