This post is a Support Worker role within Sussex Community NHS Foundation Trust (SCFT) Children's Community Nursing Service (CCNS). It is focused on the provision of respite to children with life threatening or life limiting conditions (as defined by Together for Short Lives) and their family. This will take place within community settings in conjunction with the local children's charity, The Sussex Snowdrop Trust.

Main duties of the job

The Support Worker role will help parents by taking Snowdrop children and/or their siblings out for various periods of time, or by looking after children in their own homes so that parents can have some time for other children, or themselves.

As a team we believe that children and young people have the right to be cared for at home wherever possible. This is best achieved through excellent partnership working with the child or young person, and their families or carer. The Support Worker will play an integral role in this vision.

The Support Worker will also offer clinical and administrative support where needed to the CYP Nursing team, in order to provide holistic and quality care to all children on the caseload. Currently the integrated team work over a seven day period.

Based in Chichester in West Sussex, the integrated Children and Young People's Nursing team with Snowdrop consist of Community Children's Nurses, Counsellors and Support Workers.

Job responsibilities

Main Tasks

  • Provide respite to those children/siblings on the Snowdrop pathway.
  • Accompany children/siblings to cinema/trampolining/park/bowling/outdoor activity centres etc as appropriate as part of the short breaks provision.
  • Provide age appropriate resources and activities whilst carrying out respite, enabling children to achieve and maintain identified goals eg cooking, crafts, painting, play, reading stories.
  • Provide developmental stage appropriate resources and activities whilst carrying out short breaks.
  • Maintain a folder of resources for respite activities related to different annual celebrations/festivals/seasons and use these appropriately as above. Ensure this is kept updated and rejuvenated regularly.
  • Maintain a stock of resources for indoor activities, eg baking, painting, crafts.
  • Liaise with The Sussex Snowdrop Trust in a timely manner when further resources are required to support the short break provision.
  • Maintain an awareness of all outside/external activities available to access for those children on the Snowdrop pathway and access these as appropriate.
  • Ensure regular risk assessments are carried out for outside activities, updated and recorded in patient notes as appropriate.
