Job summary
We are looking for a compassionate, motivated and positive Team Lead to join our award winning End of Life Care Hub (ECHO) team in Lancing. If you're ambitious, proactive, and experienced in Community care looking to take a lead role in a team that provides excellent care coordination to patients in their last year of life, we'd love to hear from you.
ECHO is the nurse-led Single Point of Access for referral management, clinical triage, signposting and coordination of highly effective Community-based services within SCFT for patients registered with a Coastal West Sussex GP.
The service operates 24 hours a day, 7 days a week & 365 days per year.
The team works closely & collaboratively with other services within the ECHO partnership including University Hospitals Sussex Acute Trust, St. Barnabas & St. Wilfrid's Hospices, Midhurst Macmillan Service & SECAmb, as well as liaising on a daily basis with Community Specialist Nurses and Consultants, to provide high quality care to all patients & service users.
Main duties of the job
In this role, you will be responsible for the operational leadership and coordination of the team, the post holder will have a lead role in supporting development and delivery of high quality standards within the ECHO Service.
Working with the clinical & administrative staff members, you will ensure the clinical effectiveness of the service and the delivery of an excellent patient experience. You will be part of the development of the service through reflective practice, operational management and leadership, and quality improvement initiatives.
Your experience of working across a variety of clinical settings and working autonomously will help you excel in this role. We will support you to develop specialist areas of practice and your leadership skills through internal and external programmes.
In this role you will be responsible for all aspects of staff management, supervision, and development of the ECHO team, including roster management, risk stratification, performance development reviews & play a key role in supporting the Clinical Service Manager with the transformation of the service and its staff.
