Job summary
In the week of World Diabetes Day we're excited to share this rare opportunity to support our adult Type 2 Diabetes Group Education programmes. If you're keen to work with patients to understand and overcome barriers to engagement and, importantly, empower those living with Type 2 Diabetes to make use of structured group education, we'd like to hear from you. No clinical experience is required but a background in health promotion including data analysis and an interest in diabetes are a must. This is a fixed term contract for one year - please let us know if you're interested in a secondment to our multi-disciplinary team. We offer flexible working including opportunities to work from home and full-time hours to suit the needs of the education programme so flexibility is important.
Main duties of the job
Structured group education programmes are recommended by NICE to develop attitudes, beliefs, knowledge and skills in those living with Type 2 Diabetes, their family members and/or carers. Working alongside clinicians delivering structured education, you'll be pivotal in supporting improved attendance and engagement in the population groups. You'll need to be personable with great communication skills, being open to feedback from attendees and non-attendees to encourage increased participation in programmes and events. Attention to detail is a must as is being adept in collating and analysing data to better understand the needs of the population(s) who are clinically appropriate. As part of this you'll capture patient feedback with integrity, respecting confidentiality and data protection principles. Working autonomously while collaborating with healthcare partners, you'll have opportunity to make recommendations regarding the referral and booking processes as appropriate which may include the need to develop health promotion materials. Supported by the Operational Lead and the Clinical Services Manager you'll receive regular supervision to help keep you on track throughout the year.
