Job summary

This would be a fantastic opportunity for an experienced clinical pharmacist wanting to make a positive difference to patient care by delivering medicines optimisation to older people in the community.

You will be sharing the Medicines Management leadership of the Urgent Community Response, and Short-term Bedded Units small community pharmacy teams in Brighton and Hove.

Urgent Community Response team is a short-term service offering patients in crisis the opportunity of having care and support at home with the aim of either preventing avoidable admissions into hospital, or supporting early discharge out of hospital. This role will be dynamic and innovative, working within the wider team to deliver a pharmacy service to our patients. The role will involve a mixture of office work and patient home visits.

This post will be based at Brighton General Hospital and will involve travel mainly around Brighton and Hove. This will require you to have access to your own mode of transport vehicle (e.g. car).

Working hours are Monday to Friday, there are currently no weekend, on-call or bank holiday working commitments.

Main duties of the job

The post holder will provide highly specialised pharmaceutical input into Urgent Community Response Team and short-term adult bedded units within Brighton and Hove multidisciplinary teams.

As a core member of the multidisciplinary team, you will visit adults with multiple complex long-term conditions, mainly in their own homes, and Short-term Council Bedded Units to provide patient-centred structured medication reviews (SMR) and make recommendations to improve patient outcomes focusing on quality and safety, ensuring that each patient receives the drug therapy that is correct, optimised, safe and effective for their particular situation.

You will be liaising with primary care GPs, pharmacists, community pharmacies, and other healthcare professionals involved in patient care to implement medicines interventions with the aim to prevent hospital admissions.

Liaising across different healthcare settings will be paramount to ensure effective patient transfer of care across the interface. As part of your lead role, you will identify problems that can arise for patients at interfaces and develop safe systems to resolve these.

It will be part of your role to ensure that there are safe and effective systems in place for the management of medicines through the development and implementation of procedures, training, audits, and control assurance.

The post-holder should have previous clinical pharmacist experience. Independent prescribing qualification would be desirable.
