It's an exciting time for our Urgent Community Response (UCR) Service as we expand into the High Weald, Lewes, and the Havens (HWLH) area. We are recruiting to our new teams who will work with patients in their homes across the HWLH communities.
As part of our expansion, we are looking for Care Certificate Educators to join our Practice Development Team using their expertise to develop the new teams and provide ongoing support to community and inpatient staff across HWLH. If you're an ambitious, proactive, and experienced healthcare support worker, we'd love to hear from you.
Our Urgent Community Response Service is made up of multidisciplinary teams working flexible to provide a 7-day service, 365 days a year, between 8am and 8pm. They deliver:
- A rapid response to people in the community who are experiencing acute episodes of ill-health.
- A supported discharge service to facilitate timely hospital discharges.
- Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.
The team works closely with other services within Sussex Community NHS Foundation Trust (SCFT), as well as our acute and community partners, GPs, South East Coast Ambulance Service (SECAmb), adult social care and voluntary and third sector organisations.
Main duties of the job
You will act as a mentor to Healthcare Support Workers across SCFT, including Health and Therapy Assistants in the UCR Service, to assist in competency sign-off such as patient observations and fundamental care delivery. You will play an active role in the Healthcare Support Worker training programme, and support staff new to SCFT undergoing the Care Certificate.
We are looking for candidates with specialist knowledge who demonstrate strong clinical, leadership and communication skills, are innovative and flexible, and committed to evidence-based working.
You will need to able to drive with a full UK licence due to the geographic area covered by the team.
