We have an opportunity for a Therapy Practitioner in our North Community Therapy Team based in Horsham, so if you're an ambitious and proactive individual looking to work in a team that provides excellent therapeutic care, we would love to hear from you.

This is an amazing opportunity to become a member of the Community Therapy Team providing proactive community rehabilitation and urgent care pathways to help patients stay in their homes, avoid unnecessary hospital admission and support them to live as independently as possible.

Our teams work flexibly to provide a seven-day service, 365 days a year, between 8am and 8pm. Main duties of the job

In this role, your day will be varied and interesting, managing clinical risk within your own patient case load, and supporting patients towards achieving more independence in their activities of daily living. You will be responsible for prescribing, and competent use and maintenance of equipment relevant to your service area.

You will complete non-complex MDT assessments under the direction of a registered health professional and be actively involved in MDT meetings. You will implement patient centred treatment programmes and will regularly monitor, evaluate, and modify these programmes, with clear documentation. Promoting self-management and providing education, advice and signposting will be key to delivering the best for our patients.

You will be part of our supportive team, ensuring that you feel confident and able to undertake this role to the best of your ability, whilst being able to reach your own potential through personal development along with peer and supervisory support.

You will need to able to drive with a full UK licence due to the geographic area covered by the team.
