Job summary

This is an opportunity for an experienced pharmacy technician to join our dynamic Urgent Community Response Pharmacy team.

Urgent Community Response team is a short-term service offering patients in crisis the opportunity of having care and support at home with the aim of either preventing avoidable admissions into hospital, or supporting early discharge out of hospital.

The role will be dynamic and innovative, working within the wider Urgent Community Response team to deliver a pharmacy service to our patients. The role will involve a mixture of office work and patient home visits. Flexible working and job share will be considered.

This post will be based at Brighton General Hospital and will involve travel mainly around Brighton and Hove.

Main duties of the job

You will work alongside the senior clinical pharmacists andmembers of the multi-disciplinary team in Urgent Community Response which include:Advanced clinical practitioners, nurses, occupational therapists, physiotherapists, health and therapy assistants.

You will assist the senior clinical pharmacists with domiciliary visits to patients helping to improve patient outcomes focusing on quality and safety. You will maintain effective working relationships, liaising with GP practices, community pharmacies and social services as required to help deliver medicines optimisation for patients. In addition, you will be required to provide training for the use of Medicines Administration Records (MAR charts) to the team and be able to write them as required.

You will also be working with a clinical pharmacist to deliver a high quality and effective medicines management service to our patients in adult bedded units which are managed by Brighton and Hove City Council. You will also be working alongside GPs and staff employed by Brighton and Hove City Council to deliver excellent and safe patient-centred care.

Job responsibilities

The post holder will be an integral member of the Sussex Community NHS Foundation Trust (SCFT) Urgent Community Response Pharmacy Team.

The main responsibilities are to:

  • Develop and provide specialist technical pharmacy services to Urgent Community Response and to adult bedded units in Brighton and Hove (managed by Brighton and Hove City Council) in consultation with the Senior Urgent Community Response Clinical Pharmacists.
  • To advise on the safe management (storage, handling, administration and disposal) of medicines for urgent community response and bedded units patients in accordance with medicines legislation and national and local guidance documents. This may involve influencing and supporting other healthcare professionals to produce SOPs and to participate in risk assessment of medicines.
  • To support with the supply and administration of medicines, assessing individual patients to determine their needs, and providing patient centred pharmaceutical care to patients at their own homes.
  • Contribute to the medicines optimisation agenda in the form of patient home visits with the support of the clinical pharmacist.
  • Work closely with senior clinical pharmacists to support the delivery of a high standard of medicines management and liaise with a range of other members of the MDT and external healthcare providers.
  • Provide medicines management training to members of Urgent Community Response and Brighton and Hove City Council Bedded units, and other healthcare staff as applicable.
  • Develop and comment on standard operating procedures (SOPs).
  • Plan and develop audits as part of quality improvement processes completing the Plan-Do-Study-Act (PDSA) cycle. Support and help collect data for clinical and procedural audits.
  • To produce performance data related to own work and assist the Senior Clinical Pharmacist in providing and collating information for performance monitoring and writing the reports.
