Are you an experienced clinical pharmacist seeking a new challenge, wanting to make a positive difference to patient care in the community?
Are you passionate about delivering patient centered medication reviews to older adults with complex health needs, in their own homes? If so, we are looking for highly enthusiastic, proactive and self-motivated Senior Clinical Pharmacists to join our Medicines Management Team.
Main duties of the job
You will be the medicines optimisation lead for the local ProactiveCare multidisciplinary community teams in your area.
As a core member of these teams, you will visit adults with multiple long term conditions in their own homes for patient-centred structured medication reviews (SMR), reviewing prescribing and making recommendations to improve patient outcomes focusing on quality and safety. You will be liaising with primary care pharmacists, community pharmacies and others involved in patient care as required to implement medicines optimisation.You will be supported by a small team of experienced pharmacists as well as the wider trust pharmacy team; the medicines management team (MMT). Your development and career progression is also important to us - to start with you will need to have a Clinical Diploma or evidence of robust relevant CPD, a knowledge of prescribing in the frail elderly population is desirable but not essential as we can support your development. If you are not already a prescriber, we will support you to undertake a prescribing course once established in role.
Looking to the future the proactive care pharmacy service is part of an expanding, dynamic and proactive Medicines Management team with roles across East and West Sussex including in-patient, care home, advanced and specialist pharmacy roles, so there will be opportunities to further develop your career in the future.

You will also be expected to periodically travel to other areas covered by Sussex Community Foundation Trust. This will require you to have access to your own mode of transport (e.g. car).
Multiple posts, base can be agreed from; Chichester, Bognor Regis, Pulborough, Littlehampton, Worthing
Working hours are Monday to Friday, no weekends or on call. Flexible working may be considered.
