An exciting opportunity has arisen for an enthusiastic and experienced SCPHN to work within Brighton and Hove 0-5 HCP in a new development post.
The post is initially temporary for a year with potential opportunity to extend post evaluation of effectiveness.
The Specialist HV SEND role will involve usingadvanced skills and expert knowledge to assess, refer and support the needs of children with additional developmental needs. These children will mainly be pre diagnosis. Professional development will be offered as part of your continuing development, and to help you meet the skills, knowledge required for the post.
The post holder will need to have experience of working within a variety of clinical settings, particularly community and working as part of a multi-professional team.
The post holder will need to demonstrate advanced assessment skills and experience of working with families with additional developmental needs .tra Additional skills and training in areas such as ASC and SEND would be an advantage,
As a member of the team, you will have the opportunity to:

Main duties of the job
The Post holder, working within Sussex Community Foundation Trust (SCFT) will be responsible for leading and developing the 0-5 Healthy Child Programme pathway for babies, children & young people with disabilities and additional needs pre-CDC diagnosis and co-ordinate the assessment referral and care plan for children where there is not an allocated HV. They will ensure, jointly with other services, the development of multi-disciplinary pathways, policies, and procedures to provide a team around the child & family approach to service provision.
The Post holder will have a high level of relevant expert knowledge and skill and work in collaboration with the wider multidisciplinary team such as Child Development Services, GPs, third sector organisations and the local authority.
The Specialist Health Visitor will act as a specialist resource for other staff providing expert knowledge advice and information, training and support to the universal children's services client training, groups, and forums in relation to SEND, as required. This includes providing outreach support to the families, School Nursing and Health Visiting teams

To work in a multidisciplinary setting and team to promote the needs of children with special needs, prioritizing workload and developing effective partnership working with others and play a leading role in the development of new services.
Works closely with Health, Education, Social services, and Voluntary agencies such as AMAZE and MASCOT, Community Therapists, , Health Visitors, School nurses, GPs BHISS SALT children centres and family HubsThey will provide professional leadership as a specialist practitioner to manage and co-ordinate the health needs, assessment, and referrals to CDC of children under the care of the 0-5 HCP. This may supplement the care given by the universal health visiting service throughout Brighton and Hove.
To provide clinical assessment, advice, and support, for babies and children with additional needs to families colleagues within the HCP teams, integrated team, and other agencies.
To advocate for families within the multidisciplinary team and make referrals as appropriate. Workwith others to deliver health and support services to ensure effective resource-deployment and that the services are provided in a coordinated way.
Participate in the multi-disciplinary assessment of babies, children & as appropriate. Developand maintain own clinical expertise and specialist professional knowledge about SEND, including appropriate interventions.
Work with the HCP clinical services manager for health visiting, supporting continuing development of staff and contribute to a learning environment that promotes reflective practice and continuous learning in the field of perinatal and infant mental health.
They will actively engage with and involve service users, using a variety of media including social media as appropriate, in both care delivery and service co-design ensuring feedback is used to improve HCP SEND services.
To always practice within NMC standards and the Code of Conduct Ensures the appropriate utilization of allocated resources and works within budget restraints to deliver the HCP 0-5 service which is responsive to changes in national and local policy with integrated care pathways delivered in partnership arrangements which support new models of care.
Acts as a guide and mentor to staff in the universal 0-5 HCP teams ensuring that high quality, evidence-based care is delivered to infants and their families within the HCP/SSV pathway.
Ensure that all local HCP policies and procedural guidelines relating to SEND are evidence based, appropriate and that they are adhered to.
Identify areas for research, audit, and development in HCP relevant to SEND. Initiate and participate in research and clinical audit programmes.
Maintains the safety and protection of children by following Sussex Child Protection Procedures, develops services which promote the safety and well-being of children and participates in meeting discussion and case conferences as appropriate.
In addition to undertaking assessment of children, they will have the opportunity to widen their portfolio by providing operational leadership for the service and sharing expertise with other professionals.
