Job summary

An exciting opportunity has arisen for a Community Nurse to play a key role and join our friendly team within Haywards Heath Primary Care Networks (PCN). We are based centrally in Haywards Heath covering the town and picturesque villages, providing care to those within their own homes.

The Primary Care Networks are groups of practices working together in collaboration with community and other health and care services. PCNs will enable greater provision of proactive, personalised, coordinated and more integrated health and social care. Care is organised around individuals' identified needs through collaborative working by service providers to deliver excellent patient care at the heart of the community.

Main duties of the job

The post holder will have responsibility for delivering community care to patients, including but not exhaustive of: catheterisation, end of life care, syringe driver management, sub-cut and IM injections, pressure area care and prevention, wound care, and leg ulcer care. You will also be require to attend health and social care meetings, act as an advocate for your patients, be involved in contingency planning for health, assist with supporting self-management and supporting colleagues and students with training needs, supervision and performance and development reviews.

You will be required to support with covering the hours of the service which is 8am-6pm 7 days per week. You will need a full driving licence and access to a car for business use.

The successful candidate will be hard working, flexible, be able to work autonomously as well as part of a wider team, have excellent communication skills, a good sense of humour and a desire to provide a high standard of care to our patients.
