Are you a registered general nurse with post registration experience including working with patients with diabetes in the acute or community setting?

Main duties of the job

We are seeking a motivated and skilled nurse who is looking for the opportunity to work within Diabetes as part of a nurse-led team based in the Community. In this exciting position you will have the opportunity to develop your diabetes skills & knowledge and contribute to the implementation of new initiatives including education programmes, projects, quality audits, and other service developments.

You should be a Registered Nurse, with a qualification or working towards or a willingness to undertake, in diabetes and have an interest in working with people with Diabetes.

The successful candidate will have proven clinical skills to support people with diabetes and must be able to work autonomously to deliver high quality patient care.

You will be involved in delivering nurse-led clinics in community settings for the management of complex patients including the initiation of insulin and GLP-1 injectable therapies

You will be involved in the delivery of education to both people with diabetes and healthcare professionals.

You will work alongside and build relationships with the local GP surgeries, secondary care diabetes colleagues, delivering high quality Diabetes Care across the community setting.

You will also help provide structured education to both professionals and patients.

You must be self-motivating, organised, computer literate and able to demonstrate excellent interpersonal skills with a commitment to advancing diabetes care through collaborative working with the multidisciplinary teams and personal development.

Support towards CPD will be given including attending external courses and conferences as the service allows.

The successful applicant will demonstrate excellent communication skills, autonomous caseload management skills and an ability to work flexibly within a team.

Whilst the post is advertised for full time this is flexible.
