Job summary

An exciting opportunity has arisen as part of the new Cardiff and Vale University Health Board Supportive and Palliative Care Strategy.

  • To act as a Specialty Doctor in Specialist Palliative and Supportive Care within clinical services based in Cardiff and Vale University Health Board (CAVUHB) and within services overseen by Velindre University NHS Trust.
  • To provide high quality, timely, patient-centred care for patients requiring specialist palliative or supportive care services in CAVUHB hospitals and those accessing CAVUHB front door, outpatient and community services.
  • To work to ensure an integrated model of palliative and supportive care service delivery to all patients within the CAVUHB catchment area by collaborating closely with other specialist palliative and supportive care providers as well as generalist providers.
  • To contribute to the teaching, audit, quality improvement and research activities of the palliative and supportive care teams in CAVUHB and the wider palliative care services across Wales.

Main duties of the job

Clinical - CAVUHB Hospital Support Team and Outreach Services

  • To act as a Specialty Doctor in Specialist Palliative Medicine to all geographical areas of CAVUHB.
  • To work with the Specialist Palliative Care Hospital Team of doctors, CNSs, specialist palliative care pharmacist, OTs and other team members offering clinical support, advice and peer support.
  • To provide medical expertise to support the Supportive Care multiprofessional team with clinical decision making at MDT, daily advisory support or ad hoc ward/community home visit reviews.
  • To provide specialist assessments of in- and out-patients referred by other clinical teams.
  • To work with the Palliative/Supportive Care Consultants and CNS's at UHW looking at service development, performance outcomes, clinical governance and service evaluation as required, overseen by the Clinical Lead.
  • To continue to develop and improve links between the Cardiff and Vale University Health Board, Marie Curie Cardiff and Vale Hospice, City Hospice, Velindre University NHS Trust and Primary Care Services
  • To work to achieve the clinical priorities of the Cardiff and Vale University Health Board Supportive and Palliative Care Strategy and Wales Palliative and End of Life Care Programme Board including annual national audits.

About us

CAVUHB employs approximately 14,000 staff across 5 hospitals including the main base of University Hospital of Wales (UHW) which is the tertiary teaching hospital for Cardiff University. UHW is the regional centre for Neurosurgery, Cardiothoracics, Transplant, Liver and Spinal Surgery, Teenage and Paediatric Oncology for South Wales and a Southeast Wales specialist centre for renal, haematology, neurology, cardiology and head and neck and gynaecological cancers. It has nearly 2000 beds and is also a DGH for CAVUHB residents. The Specialist Palliative Care hospital teams are based in UHW and University Hospital Llandough (UHL) and receive over 2000 referrals each year from all directorates within the hospitals.

The Hospital Palliative Care team is a well-embedded service within the two main Cardiff Hospitals and provides palliative care to in-patients via a multiprofessional team to all clinical areas.

The Supportive Care team is a new service model which successfully redresses inequity of access to palliative input in non-cancer life-limiting conditions. It consists ofamulti-disciplinary team providinga cross-boundary, patient-centred approach. This has overcome referral barriers through a co-speciality approach encouraging earlier upstream referral.
