Clinical Sister/Charge Nurse

Job summary

The post holder is responsible for the assessment of care needs, thedevelopment, implementation and evaluation of programmes of care,participating and contributing to the wider clinical governance agendaincluding service developments and flexible approach to meet the needsof patients.

Managing a case load of patients receiving a range ofOncology treatments and management of the associated complications.

There is also a requirement to supervise junior staff and teach qualifiedand unqualified staff including basic and post basic students.

Please be aware that you may be required to cover in ChemotherapyService, Outpatients or Inpatients.

Main duties of the job

To lead and be directly involved with clinical and operational procedures with patients, carers and relatives.

To undertake assessments and direct care plans for patients, relatives and carers, with the team for those needing in-patient care, day-care and respite support.

To work closely with patients and their families.

To organise and lead case conferences and multi-agency meetings for patients, carers and families (particularly those with complex needs).

To ensure that all care plan requirements are met within specified time-scales.

To plan with the patient a leave or discharge plan that is followed through for each individual involving both nurses, community services and other agencies where appropriate.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
