Job summary

An opportunity has arisen for a dynamic and driven, highly motivated first level registered nurse to join the Ambulatory and Assessment Unit at team at Velindre Cancer Centre as a 12 month fixed term Band 5 role.

This is an exciting opportunity to be a part of an evolving SDEC/Semi-elective service working alongside the Acute Oncology Advanced Clinical Practitioner team.

This service is currently undergoing a period of transformation to meet the increasing demands and continued development of services. As part of this change, we are looking for passionate and adaptable candidates who thrive in a fast-paced, high-acuity environment. The service is moving towards a 12-hour 7-day provision, so applicants must be flexible and able to accommodate the changing needs of the service. The ideal candidates will have experience in managing complex care needs, including the care of the acutely unwell or deteriorating patient.

Main duties of the job

To work with patients and their carers to determine in partnership the assessment of patient needs. Working collaboratively with the multi disciplinary/multi agency team.

To liaise and provide guidance on your area of practice to inpatient and community service on the care of patients when requested.

To lead or contribute to the development of a care plan to meet the holistic needs of the patient and carer which identifies interventions, goals and review dates.

To act as the named nurse for designated individuals

To ensure that patients receive nursing care according to plan, delegating the care to be given to other members of the team as appropriate.

To plan with the patient/team a discharge plan that is followed through for each patient, involving both carers, community services and other agencies when appropriate.

To contribute to the creation of a caring environment that values patients/carers opinions and ideas and where motivation amongst staff to develop is a high priority.
