Job summary
The post holder is responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care, participating and contributing to the wider clinical governance agenda including service developments and flexible working to meet the needs of patients.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
This post is fixed term/secondment for 12 months. If you are interested in applying for the secondment position, you must obtain permission from your current line manager prior to applying for this post.

Main duties of the job
To work with patients and their carers to determine in partnershipthe assessment of patient needs. Working collaboratively with themulti disciplinary/multi agency team.
To liaise and provide guidance on your area of practice to inpatient and community service on the care of patients when requested.
To lead or contribute to the development of a care plan to meet the holistic needs of the patient and carer which identifies interventions,goals and review dates.
To act as the named nurse for designated individuals
To ensure that patients receive nursing care according to plan, delegating the care to be given to other members of the team asappropriate.
To plan with the patient/team a discharge plan that is followed through for each patient, involving both carers, community servicesand other agencies when appropriate.
