We have an exciting opportunity for a Band 4 qualified Assistant Practitioner, to come and join our Corsham, Calne and Box community team at Wiltshire Health and Care.
Community Teams within Wiltshire Health & Care provide high quality, complex care in the community setting, providing Occupational and Physiotherapy for some very vulnerable people, often in challenging environments.

Working with us will provide you with the opportunity to be part of an experienced team, working in a busy but supportive environment. You will be supporting the multidisciplinary team to ensure the health and rehabilitation needs of the local population are met, so excellent communication skills are essential for this role.
Experience in front line NHS community patient care is not essential as we are keen to attract newly trained as well as experienced assistant practitioners.

Main duties of the job
The aim of the role is to support and work alongside the Occupational and Physiotherapists to provide a rehabilitation service for people in their own home holistically, so they are empowered to control their health and recovery journey. You will also be part of the team who supports patients with their long-term and short-term physiotherapy goals and exercises. You will work closely with the therapists for assessment, care planning, patient review and MDT working.
Calne, Corsham and Box Community Team is a busy team caring for patients registered with a Calne, Corsham, Box or Colerne GP, you will have the opportunity to work alongside other members such asOccupational Therapists, Physiotherapists, Assistant Practitioners, Support Workers and other Practitioners. Our Shift patterns are 8:00-16:00, 7 days a week and you would work approximately every 4th weekend.
