Community Physiotherapist - Band 5

Trowbridge Community Team

37.5 Hours per week - Permanent

Trowbridge Community Team is looking for a Band 5 Physiotherapist to work full time in our dynamic and professional team.

We are a large, friendly multi-disciplinary team whose aim is to help individuals remain in their own home, reaching their maximum potential. From triage to urgent call outs, rehabilitation following a hospital stay and prevention of hospital admissions, you will work alongside our existing therapists and support workers to provide timely and skilled assessment and treatment to our diverse range of patients with physical health conditions.

Trowbridge Community Team has a well-established falls service, receiving referrals for individuals who have fallen or are at risk of falls. The successful applicant would primarily work with the falls service, completing falls triage, home assessments and participating in the delivery of the falls and balance groups in the community. This is an excellent opportunity to develop your skills and knowledge around falls prevention and treatment and be involved in developing this already excellent and respected service.

The post-holder will work in patients homes and residential homes and work professionally in a multi-disciplinary team, alongside Community Nurses, Occupational Therapists, Physiotherapists, Clinical Leads, Clinical Care Coordinators and Support Workers. You will also work with the GP Practices in the local area.

Main duties of the job

The team is based at Trowbridge Community Hospital. You will be expected to work shifts including some weekends and bank holidays.

Applicants need to have good clinical skills, well developed communication skills and be able to work under pressure. The role is demanding but rewarding, and we are looking for an enthusiastic, motivated and hardworking individual who can work flexibly and efficiently.
