Specialist Community Nurse - Band 6

Trowbridge Community Team

37.5 Hours per week - maternity cover

If you love working in a Community setting we have a great opportunity for a Band 6 Community Specialist Nurse to join our motivated and proactive Trowbridge Community Team. We would also be prepared to consider this as a developmental role for a Band 5 Nurse wishing to commence the Specialist Practitioner District Nurse Qualification (SPQ) pathway or a Nurse who has already completed the SPQ programme. This post is currently 37.5 hours over 5 days. This is a fixed term opportunity due to Maternity leave of our band 6 Nurse and will be for 12 months.

The team is based in Trowbridge Community Hospital and provides patient care between 07.00 22.00 hours with shift patterns including some weekends, early and late shifts which can be flexible. The team supports patients who are registered with GP practices in the Trowbridge locality, and we work closely with the GPs and the wider Multi-Disciplinary team including our Therapists, Social Care and other agencies.

This role is very rewarding and will include supporting a caseload of patients and providing effective nursing interventions in patients homes and other community clinical settings using remote working technology. As a Community Specialist Nurse you will be involved in caseload and case management, managing complex patients and coordinating shifts.

Main duties of the job

The role includes supporting other team members including providing clinical advice and support alongside supervisions and some HR processes. Other aspects of the role include managing complex CHC patients, attending MDT meetings and a wide range of other clinical interventions. Our caseload includes patients with palliative care needs, simple and complex tissue viability issues and long term conditions.

If you would like to join our team you will need some experience in NHS community patient care, evidence of up-to-date clinical professional development alongside excellent clinical and communication skills.
