Corsham/Calne Community Team

37.5 Hours per week - Permanent

Home based therapy is the programme for the future. Following a period of hospital care we know that patients progress better in their own home environments and with a tailored programme of rehab we are best placed to support them.

In the North of Wiltshire, we would like to recruit a Home First (HF) Assistant practitioner or Developmental Assistant Practitioner for the Calne and Corsham Community Team. The successful candidate will work closely with our locality HF Co-Ordinator, therapy team, the Wiltshire Health and Care Locality Hub and the wider multi-professional teams.

Main duties of the job

The role will be based in Calne Family Health Centre, with expected travel to support across our split site team in Calne and Corsham. You would be required to manage Home First referrals, consider the patients need and allocate care plans that support the Home First programme for each patient. This will include the need for therapy initial assessments and support worker input. You would be required to ensure that patients receive regular reviews, monitor onward referrals, and promote flow through the service. Your role will be central in ensuring that the service runs efficiently and meets the needs of the patient.

The successful candidate would also need to be literate in SystmOne, able to communicate clearly and concisely. Experience of the community team, therapy services and Home First is desirable but not essential.
