Strategic Clinical Networks are an essential national vehicle for joint working between key stakeholders including NHS Wales, Welsh Government and the third sector to improve the outcomes and experience for the population of Wales. Integral to this is the need for continuous improvement and transformational change across a range of our services and approaches and the role of the Strategic Clinical Network is to support this at national, regional and local levels.

This role provides an excellent opportunity for a highly motivated individual to support the national delivery of the National Strategic Clinical Network (NSCN) for Diabetes.

The post holder will be a health professional working within the speciality of psychology within diabetes care services with in-depth clinical knowledge and experience.

Main duties of the job

  • To provide clinical leadership to, and on behalf of, the SCN for Diabetes.
  • To foster a culture of multi-professional engagement in the improvement of care across integrated pathways for patients with Diabetes. This will include ensuring Psychology colleagues are fully committed to the delivery of ambitious programmes of pathway redesign and quality service improvement.
  • To ensure involvement, influence and distributed leadership is promoted and available across the care system.
  • To maintain credibility with all key players within the Diabetes community, fostering a culture of collaboration for the delivery of equitable, high-quality care across all sectors. At times this will include acting as an 'honest broker' reconciling conflicting views and interests.
  • To understand, communicate and support the implementation of clinical policy relating to Diabetes (including Quality Statement, NICE Quality Standards and Outcome Strategies).
  • To promote a culture of innovation, developing and maturing strong partnerships across the Diabetes landscape.

All Wales Diabetes Implementation Group was established 2014 to improve the care received by people living with and caring for those with diabetes across Wales. On the 1st of October 2023, the All Wales Diabetes Implementation Group became the National Strategic Clinical Network for Diabetes in line with the National Clinical Framework (National Clinical Framework - NHS Wales Executive). The Diabetes Strategic Network will focus on providing a greater strategic direction for diabetes care across Wales.

Building on the work of the 2023 Diabetes Quality Statement, and supporting development of future iterations, the National Strategic Clinical Network (SCN) for Diabetes will provide national leadership and drive forward change to deliver better quality, higher value, more consistent and accessible services for people affected by Diabetes. This should take into account psychological support required for patients, but also workforce challenges and the impact and opportunities of innovation and different ways of working.

The post sits within the SCN for Diabetes and NHS Wales Executive which supports and advises Health Boards and Welsh Government in Wales. Whilst the main responsibilities will be to the new SCN for Diabetes, the post holder will be required to work flexibly to respond to the business needs of the supporting clinical reference groups and workstreams consistent to those described in this job description.

The post holder will work with the National Strategic Clinical Network (SCN) Clinical Lead, Network Manager H and core team to develop models of care closer to home, ensuring pathways are implemented to deliver a consistent integrated delivery of care at the point of need. This will also require the postholder to work across other National Networks and Programmes (in particular Mental Health) to ensure a whole system approach is met, reducing potential for silo working.
