Job summary

An opportunity has arisen for experienced nurse to join the multidisciplinary diabetes team in Portsmouth.

Working autonomously, but as part of a progressive and supportive multidisciplinary team, the post-holder will provide clinical care and leadership for diabetes services across the acute hospital Trust, linking with community services to support seamless patient care.

Areas of clinical service provision across the team are: Care of inpatients with diabetes, Type 1 diabetes services (including use of emerging technologies and education to support this), young persons transition from paediatric services, management of diabetes prior to and during pregnancy, renal and foot services.

Main duties of the job

  • An autonomous practitioner, responsible for the assessment, primary and/or secondary diagnosis, management, and evaluation of treatment and/or care for a caseload of patients.
  • Coordinate and implement an overall strategy of evidence-based care for a caseload of patients.
  • Lead and coordinate the service alongside the lead consultant.
  • Through effective leadership ensure the clinical multidisciplinary team deliver a safe, effective, responsive, caring and well-led service where compassion is embedded, and a positive patient experience is achieved.

Clinical/Professional

  • Promote and set high standards of professional nursing care and maintain the delivery of quality care within the trust policies, protocols and guidelines.   • In collaboration with members of the multidisciplinary team, ensure that the users of the service and their families have a positive experience and using feedback (Elephant Surveys, Family and Friends Test, concerns and complaints) evaluate patient processes and pathways of care are redesigned as required.
  • Ensure concerns raised by patients and their families are dealt with in an open, honest and transparent way, quickly and effectively, in line with trust policies, procedures and Duty of Candour legislation.
  • Ensuring that accurate and timely MDT records are maintained, monitoring standards and taking corrective action as required.
  • Ensure the MDT are competent in the use of any medical devices/equipment and have access to training and monitoring compliance.
