Job summary

An exciting opportunity has arisen for a new post within pharmacy services to Gastroenterology and we are looking for an enthusiastic and self-motivated pharmacist to join our well-respected and friendly team.

You will work as part of a dedicated multidisciplinary team to deliver and develop specialist pharmacy and medicines management services to the Gastroenterology department, including both inpatient and outpatient services and will work closely with our specialist Inflammatory Bowel Disease team.

This is a fantastic opportunity for career progression and will be well supported by the lead pharmacist for Gastroenterology and Hepatology.

Main duties of the job

To support the lead pharmacist for Gastroenterology and Hepatology to develop and coordinate pharmacy and medicines optimisation services.

The post-holder will:

  • Act as a specialist role model and ensure a high level of clinical pharmacy input to the care of patients within Gastroenterology.
  • Ensures the continuity of pharmaceutical care by provision of drug history, prescription monitoring, clinical monitoring of drugs and discharge planning.
  • Work directly with clinicians, specialist nurses and other allied healthcare professionals to deliver safe and effective treatment.
  • Actively participate in multidisciplinary team meetings within Gastroenterology.
  • Act as a specialist role model and maintain excellent working relationships with all those that use, commission and work within the services delivered.
  • Support the lead pharmacist with horizon scanning and financial management of medicines expenditure including the use and funding of new medicines.
  • Provide specialist education to patients and carers.
  • Develop, implement and audit local guidelines.
  • Provide education and training to a range of healthcare. professionals.

The post holder will:

  • Be responsible for delivering specialist clinical pharmacy and medicines management services to the Gastroenterology Department including the Gastroenterology ward and Gastroenterology Outpatients, to support the Lead Pharmacist for Gastroenterology/Hepatology.
  • Advise consultants, junior doctors, nursing and pharmacy staff on all aspects of medicines for gastroenterology patients.
  • Recommend, substantiate and communicate best therapeutic options for patients and implement changes in prescribing practice to ensure evidence-based medicine is followed as appropriate under the guidance of the Lead Pharmacist.
  • Undertake clinical audit of medicine use and pharmacy services within Gastroenterology.
  • Ensure compliance with medicines legislation, the Trust Medicines Management Policy, the Trusts Controlled Drugs Policy, and related national and local medicine guidelines.
  • Provide clinical pharmacy teaching to medical, nursing and pharmacy staff.
  • Monitor drug expenditure within gastroenterology under the guidance of the Lead Pharmacist including the monitoring of the use and funding of new medicines.

  • Provide clinical ward cover and clinical pharmacy expertise in use of medicines to and for patients within the Gastroenterology Department during an inpatient hospital stay or on an outpatient or day case basis.
  • Evaluate prescribing practices and amend according to evidence-based practice, national/regional directives and clinical governance targets on a daily basis, during Consultant ward rounds and at multidisciplinary team meetings.
  • Ensure all newly admitted patients are interviewed to enable an accurate medication history and subsequent medicines reconciliation.
  • Participate in the use of Patients Own Drugs schemes within the specified clinical area, where appropriate.
  • Assess patients discharge information requirements as early as possible during the in-patient admission so as to be able to prepare information on medicines based on their individual need.
  • Provide advice on pharmaceutical matters within the specified clinical area, undertakes risk management and ensures compliance with medicines legislation and other national guidance.
  • Promote and contribute to clinical audit, research and development, to provide information to managers & clinicians on the quality and effectiveness of the clinical pharmacy within the specified clinical area and improve the provision of pharmaceutical care.
  • Develop guidelines and processes to enable the safe, efficacious and cost-effective use of medicines within the specified clinical area, to include cost control and reduction where necessary.
  • Support clinicians through the preparation of prescribing guidelines for the relevant medicines management committees to ensure that medicines are used in such a way as to maximise benefits to patients within available resources.
  • Provide comment on drug expenditure with the specified clinical area on a monthly basis to the directorate pharmacist and team as required.
  • Provide clinical pharmacy training, education and support to pharmacy staff to meet the needs of the individual and the service to the specified clinical area. Provides training and support for junior doctors and nurses in specific aspects of medicines management.
  • Provide tutor support to a pre-registration trainee pharmacist, student undertaking the postgraduate certificate in clinical pharmacy, newly qualified pharmacist as appropriate.
  • Support pharmacy quality systems through the development of work instructions for the area of expertise.
  • Supervise the dispensing of medicines within legal and ethical requirements and in accordance with local guidelines and work instructions to ensure that patients receive the correct medication in a timely manner.
  • Provide out of hours service through the on-call rota.
  • Support colleagues within the pharmacy team including pharmacy technicians and assistants in the delivery of a robust clinical pharmacy service.
