Job summary

If so, a fantastic opportunity has arisen to join the Portsmouth Enablement Centre (PEC), located at St. Marys, as a Prosthetics Clinical Nurse Specialist. Portsmouth Enablement Centre provides a regional prosthetic service to people living in Portsmouth, Southampton and wider Hampshire, as well as some areas of West Sussex.

It is one of over 40 such centres across the UK, including one of nine enhanced veterans care centres which provide specialist prosthetic and rehabilitation services for veterans whose amputation is attributed to their military service. This role is a twelve month fixed term contract.

Main duties of the job

  • The assessment, management and ongoing evaluation of patients following limb loss. This includes supporting patients along the entire age group from pre-natal support, children and adults.
  • Ongoing assessment of patient's wounds post operatively, prior to and during prosthetic rehabilitation and ongoing once patients have then been issued with a prosthesis, as required.
  • Acting as an expert resource in wound care to ensure patients can continue with rehabilitation.
  • Working autonomously for two days per week as well as working alongside the clinical nurse specialist lead supporting the team with assessment of patients post trans-tibial amputation.
  • Working alongside the consultant and multidisciplinary team to support patients following upper limb and trans-femoral amputation.

Clinical/Professional

  • Promote and set high standards of professional nursing care and maintain the delivery of quality care within the trust policies, protocols and guidelines. Implement the trust values and beliefs ensuring they are embedded into everyday practice and evaluate their impact on the patients experience.
  • Provide professional nursing expertise to MDT within sphere of clinical responsibility. This includes working alongside the clinical nurse specialist to run nurse-led Primary Amputee clinics and working alongside the Consultants to support them to run consultant led primary amputee clinics, including supporting patients prior to their amputation.
  • Continuing responsibility for the assessment of patients following their rehabilitation journey and ongoing yearly reassessments.
  • Provide advice, support and education to patients utilising appropriate resources based on the needs of the patient and their family.
  • Take an active role in the MDT triage process of all new patient referrals to ensure the 18 week target is met.
  • Carry out vascular assessments of patients, using a hand held doppler, gain consent for and triage onward referral, as appropriate.
  • Provide advice and support to patients in line with management of the Diabetic foot, gain consent for and triage onward referral, as appropriate.
  • Assess patients quality of life by use of outcome measures, ensuring that these are consistently recorded prior to and post rehabilitation and to be able to ascertain where further support from the MDT may be required.
  • Signpost patients to other health care professionals within the MDT, based on their needs ie Consultant, Physiotherapist, Occupational Therapist, Counsellor, Prosthetist.
  • Refer patients to members of the wider team, ie Vascular Team, General Practitioners, Practice and Community nurses, Podiatry.
  • Ensure that any patient quality and safety issues are addressed in a timely manner.
  • Ensure correct handling, storage and disposal of sharps
  • Communicating, influencing and working with the multidisciplinary team in different ways to improve quality of care, and informed clinical decisions. Promote innovation and participate in quality audits to measure effectiveness of current practices.
  • In collaboration with the Clinical nurse specialist and members of the multidisciplinary team, ensure that the users of the service and their families have a positive experience and using feedback evaluate patient processes and ensure pathways of care are redesigned as required.
  • Ensure concerns raised by patients and their families are dealt with in an open, honest and transparent way, quickly and effectively, in line with trust policies, procedures and duty of candour legislation.
  • Ensure the MDT are competent in the use of any medical devices/equipment (ie glucometer, sphygmomanometer, pulse oximeter, thermometer)
  • Order PPE, dressings and clinical supplies and support other members of the MDT to ensure that correct infection control guidelines are adhered to.
  • Undertake hand washing and cleaning audits once a month.
  • Develop and maintain strong, collaborative links with the Vascular team, community teams, primary care trusts.
  • Take an active role in chairing of Governance Meetings and participate in team meetings
  • Contribute to the safeguarding of children through acting on any child protection suspicions or concerns and escalating as appropriate in line with the PHUTs Safeguarding Vulnerable Adults, Children and Young people; Practice Guidance/Operational Policy and was not brought policy

Research and Education

  • Support a research culture within the MDT team to identify current knowledge and deliver evidence-based care.
  • Organise bespoke training for other allied health professionals within the MDT
  • Liaise with Clinical nurse specialist/matron regarding development opportunities

Communication and Working Relations

  • Communicate highly sensitive and complex information with empathy, ensuring that information is understood.
  • Provide patients and their families with information about the amputee rehabilitation process and the open wound pathway, as required.
