An exciting opportunity has arisen to join the busy and progressive Stoma Care Department at Queen Alexandra Hospital. Due to restructuring of the team, we have opportunity to recruit a new Band 7 position to lead the team and develop the team in the future. If you have experience as an existing Stoma Care nurse then we will be keen to hear from you.
The new post holder will be responsible for the management of the existing team and actively take part in service development of the service and the team members. As the team leader, the service requires the post holder to have the Clinical Acumen and skills to guide and support all the team members and support the patients through their journey.
We are currently running a 7-day week service to provide continuity and support for patients and their support network to promote timely discharges. The service runs Nurse-led clinics and is responsible for the follow-up of patients post discharge as well as counselling patients prior to the formation of Stomas for complex cancer surgery benign conditions.
Main duties of the job
- An autonomous practitioner, responsible for assessment, management and evaluation of treatment and/or care for a caseload of Stoma care patients.
- Through effective leadership, ensure the clinical multidisciplinary team deliver a safe, effective, responsive, caring and well-led service where compassion is embedded and a positive patient experience is achieved.
- Provide a strong professional clinical and managerial leadership and act as a professional role model.
- Work across organisational and professional boundaries.
- Support and run Nurse-led pre and post operative clinics.
- Point of contact and support for Stoma care patients.
- Prevent admissions and facilitate early discharge.

Clinical/Professional
- Act as a specialist clinical practitioner, in a variety of settings (ward/clinic/virtual)- demonstrating expert clinical skills and knowledge in the field of stoma care in line with trust policies, protocols and guidelines.
- Provide stoma care teaching and support to patients whilst upholding the Trust's 'values and beliefs', ensuring that they are embedded into stoma care practice.
- Proactively support patients by providing ongoing support, empathy, and reassurance throughout their recovery and life with a stoma.
- Make independent clinical, psychological and social assessment of patients and initiate/interpret appropriate evidence-based investigations/treatments as required.
- Delivering quality stoma care both independently and as part of the team, prioritising workload to ensure patients are seen and any issues are resolved in a timely manner.
- Act as an advocate and assist patients to make informed decisions concerning their treatment (use of suitable products etc.) by providing expert advice and guidance.
- Provide high quality verbal and written information to patients, families, carers and members of the multidisciplinary team. Make use of the resources available (interpreters, written documents etc.) for service users who do not speak English. Ensure to confirm understanding wherever possible.
- Lead the wider multidisciplinary team by providing expert advice in relation to care plans and discharge planning for stoma care patients, ensuring that patient safety is at the centre of any decisions made.
- Ensure the multidisciplinary team are competent in the delivery of basic stoma care through leading on the delivery of relevant training and monitoring of compliance.
- Reduce emergency admissions by safely providing timely telephone advice to patients and their families. In addition, provide education to health care professionals both in hospital and in the community.
- Accept referral and refer patients to other health care professionals/external companies as required in the hospital and or in the community.
- Ensure that any patient quality and safety issues are addressed in a timely manner. This includes monitoring and evaluating standards of care by the clinical team and providing regular feedback to the specialty team on the findings.
- Communicate effectively with all members of the multidisciplinary team involved in patient pathway to ensure quality of care is maintained throughout different areas. Ensure that communication is clear, accurate and timely.
- Lead in and promote participation in benchmarking and quality audits to measure effectiveness of current practices and development of the department wherever necessary.
- Ensure concerns raised by patients and their families are dealt with in an open, honest and transparent way, quickly and effectively, in line with Trust policies, procedures and Duty of Candour legislation.

awareness of national and local stoma care guidelines (e.g. Association of
Stoma Care Nurses UK) and influence departmental developments in accordance
with these.

contribute to, or lead on the development and dissemination of evidence-based
guidelines, standards and protocols as required and implement change to improve
the quality of the stoma care service.

to trust peer reviews and any focused CQC style reviews where required.

contribute to achieving the clinical governance goals/targets set by the trust
and department and comply with reporting schedules for monitoring and reporting
on clinical outcomes. Includes auditing

proactive in the prevention and management of complaints (formal and those via

continued work with stoma link nurses, in conjunction with their line manager, to
enhance patient experience where possible.
