Job summary
We currently have rotations in Emergency Care, Medical Village,
Surgery, Older Persons Medicines, Haematology / Oncology (We are a Cancer
Centre), MSK and Orthopaedics, Clinical trails, Renal (Wessex Renal Transplant
Centre), Cardiology, Gastro, Respiratory, Women's and Children's, Critical Care
(24 ITU beds).

Main duties of the job
- Develops and
co-ordinates pharmacy and medicines optimisation services to designated ward
areas.
- Responsible for the pharmaceutical patient care and assessment of the
pharmaceutical needs for patients on designated wards under named consultants.
- Undertakes prescription monitoring and provision of pharmaceutical care plans
ensuring the active involvement of patients and carers.
- Ensures the continuity
of pharmaceutical care by provision of drug history, prescription monitoring,
clinical monitoring of drugs and discharge planning.
- Supports patients and carers
to understand their medications using advanced communication skills,
participating in potentially emotional/distressing conversations.
- To provide medicines optimisation and medicines reconciliation
services to achieve department priorities and targets on a designated ward or
group of wards as decided by the Lead Pharmacist Clinical Services.
- To act as a
senior pharmacist within designated division/ directorate, providing specialist
advice and recommendations to other healthcare staff including senior medical
staff, general practitioners and nursing staff on matters related to the use of
medicines.
- To act as a role model for junior clinical pharmacists and support
them with their rotational objectives and further training courses.

- Participate in the provision and development of
clinical pharmacy services to divisions under the leadership of the Lead
pharmacist for the area currently working in.

Provide a patient focused pharmacy service to
the wards and departments, which may include individual patient counselling,

Independently evaluate the clinical
appropriateness of medicines prescribed for patients, using critical appraisal
skills and professional judgement and support formulary work within specialities.

Assisting the clinicians in the preparation of
business case applications for the District Prescribing Formulary to the
Formulary and Medicines Group and/or Area Prescribing Committee, and where
necessary taking cases of need to purchasers to ensure that funding for new
products or approaches to treatment is secured in contracts.

Supporting on the production of prescribing
guidelines (e.g. local / shared care / PGDs) to ensure that medicines are used
safely and in such a way as to maximize benefits to patients within available
resources.

Advise medical staff in non-formulary requests,

relevant commissioner.

Providing
training, teaching and support to underpin CPD in the Pharmacy department and
deliver general and specialist pharmaceutical education to other healthcare

competency-based assessments of band 6 pharmacists, pre-registration trainees
and pharmacy technicians, using work-based assessment tools.

When required contact General Practitioners,
community pharmacists, practice nurses and other healthcare workers in the

re-admissions due to pharmaceutical issues.
