Specialist Nurse (Integrated Community Stroke Service)

Job summary

We are excited to offer a unique opportunity for a Band 6 Nurse to join the Integrated Community Stroke and Neuro Service (ICSNS). This is a newly created role, introducing specialist nursing into our established multidisciplinary team for the first time.

Our service supports individuals following stroke or a new neurological diagnosis, enabling a smooth transition from hospital to home. We are committed to delivering high-quality, patient-centred rehabilitation within the community, helping people maximise recovery, independence, and wellbeing.

As part of a dynamic multidisciplinary team, including physiotherapists, occupational therapists, speech and language therapists, psychologists and rehabilitation support workers, you will play a key role in delivering holistic, coordinated care. You will provide specialist stroke nursing within community settings, supporting patients to work towards personalised rehabilitation goals through a combination of home visits, virtual reviews and group consultations.

A central part of the role will involve supporting secondary prevention, including blood pressure monitoring, medication adherence and lifestyle interventions. You will also contribute to fatigue management, continence care and provide support to patients, carers and families as they adjust to life after stroke.

You will work closely with an established nursing team in the Eastern locality and receive supervision and support from the Clinical Nurse Manager for Stroke, contributing to the development and delivery of services within the Northern locality.

The ICSNS team works collaboratively with short-term services, local health and social care teams, and third-sector partners such as the Stroke Association to ensure rehabilitation is seamless, integrated and focused on the needs of each individual.
