We need a highly motivated, enthusiastic individual to join our dynamic and proactive team. Applicants will require good assessment, clinical and communication skills in order to deliver evidence-based nursing care to patients within their home environment, recognising when a patient is deteriorating and intervene and escalate concerns appropriately to optimise quality of life and reduce the risk of unnecessary hospital attendance. The successful candidate will be able to demonstrate the ability to provide a high standard of care and have good communication skills. Applicants must be well organised with a professional and approachable manner. They must be prepared to work as part of a team and autonomously.

Working Pattern: 7.5hrs daily, negotiable start time. 2 FULL TIME ROLES AVAILABLE.

Please note, one fixed term contract is until 15/01/2027, and the other is until 12/02/2027. For current NHS employees this post could be a secondment opportunity.
