The successful clinician will be working in closely with the Community Practice Education Team, Care Services, Voluntary Services and Community Teams.

You will deliver proactive palliative and end of life education, management and review across community teams, care homes and external organisations.

• Be involved in Palliative and End of Life projects within the Community Services and Care Services.
• Develop and build effective working relationships with key agencies; identifying those in their last years, months or days of life.
• To provide advice to other colleagues on Palliative and End of Life Care following Trust policy, ensuring the promotion of advanced care planning and TEP form review.
• Support Community colleagues to reduced hospital admissions at end of life, by advocating preferred place of death and ensure correct skills to keep the patient in their place of residence.
• To collate, analyse and present data in relation to admission avoidance and concordance with preferred place of death auditing.

You will be part of a dedicated team, delivering a wide range of clinical education, service improvement, and involvement in safeguarding concerns. Candidates will need to have excellent communication skills; as you will be delivering education and advice to those in a variety of settings and backgrounds. These conversations may be tough at times. You will be working with wider services also, to ensure integrated working across Devon with our wider service colleagues.

We have a supportive and dynamic management team who truly value staff wellbeing and development. You will be provided with regular supervision and support with ongoing professional development as identified in your annual appraisal.

Candidates will need to own their own car, with business insurance as the role involves driving to visit settings and teams across Eastern, Mid and Northern Devon. Expenses are paid as per Royal Devon Policy.

Working Pattern: 37.5 hours per week. Monday to Friday 08:30-16:30, weekends/evening occasional if service requires only. No Nights. Flexible working pattern can be discussed at interview.

This is a fixed term contract for 12 months. For current NHS employees this post could be a secondment opportunity.
