Are you a dedicated nurse who always strives to give the highest standard of holistic care to your patients? Do you have a particular interest in palliative care?

We are seeking a motivated nurse who would like to develop their knowledge, skills and experience of working with patients with palliative care needs. You will have the opportunity to learn and gain experience through clinical reflective practice and continued professional development. You will be supported in this post by the Seaton Hospice at Home palliative care nurse specialists and the palliative care nurse team manager. The post is for a Band 6 Clinical Nurse Specialist but a developing Band role would also be considered.

This is an exciting opportunity to be part of a professional, specialist palliative care team, who work alongside members of the wider multidisciplinary team of health and social care colleagues both in the community and the acute care sectors.

The Seaton Hospice at Home is an Integrated Model for Palliative and End of Life Care. This model integrates the available services within the community into a one-team approach to provide the best possible care for all patients who are diagnosed by their GP as palliative, regardless of diagnosis or complexity, guided by the individual needs and wishes of the patient and their loved ones. This service provides continuity of care throughout the patients journey, enabling people to die in their preferred place, which is often their home.

The Seaton Hospice at Home Service is managed by the Royal Devon and Exeter NHS Trust and funded by the Seaton League of Friends. The service provides specialist palliative care 7 days a week, 365 days a year, with shifts covering 08.00-16.00, 10.00-18.00 and 12.00-20.00. After 20.00hrs an RN will be on-call to respond to our patients who need advice or a visit overnight, thus providing specialist end of life care and carer support 24/7, avoiding unnecessary and unwanted hospital admissions.

Working Pattern: 30 hours per week. 4 days a week; Shifts vary between 08:00-16:00; 10:00-1800;12;00-20:00 with an on call overnight
