Role Summary

You will be based at Barnstaple Health Centre, co-located with Social Care and Community Nursing teams, promoting a timely and successful journey for our shared patients. Your work will be delegated to you by registered therapy team members. Together with the registered team member you will work with the patient on their goals.

You will support patients with a range of interventions including mobility and home exercise programmes, small equipment aids, and functional tasks around the home.

A typical day involves catching up with the team, planning and attending community visits as per the patients requirements and documenting all contacts. Some visits will be lone working while others will be joint visits either with therapy or the wider team.

My Care is our electronic patient recording system, this allows you to record live at the point of seeing the patient using your NHS mobile phone as well as on your laptop back in the office.

Your daily routine will involve regular communication with other health and social care professionals and wider partners including Millbrook (our equipment provide), the Independent Living Centre, Care Home Team, GP's, the Community Response and Recovery Team and One Barnstaple (voluntary sector).

Our shared role with these agencies being to support our patients and their presenting needs safely within their own home/environment.

Pay:The salary advertised for this position reflects the Agenda for Change 2024-5 rates, inclusive of a 5.5% pay award implemented from October 2024. If you join before this date, the pay award will be backdated to April 2024 or your start date, whichever is later. Until then, remuneration will be based on the 2023-4 pay bandings (Pay scales for 2023/24 | NHS Employers).
