Job summary
We are looking for an experienced, innovative and flexible Pharmacist to join our team and enhance our ability to provide pharmaceutical support for patients receiving care in Eastern Devon Community Services.
- To provide a high-quality Specialist Clinical Pharmacy Service to patients in their own home and within Community Hospitals as part of an Integrated Team.
- Develop relationships with other Pharmacy Professionals and members of the Multi-disciplinary team to support integration across Health and Social Care including Primary Care, Community Pharmacy, Secondary Care, Acute Hospital at Home and Mental Health.
- Ensure safe, effective and economic medicines use to strive for best outcomes for patients.
- To be responsible for linking with the wider Health and Social Care Teams to optimise medication regimes and Pharmaceutical Care.
- Support Pharmacy Technicians with Clinical prioritisation on a day to day basis.
- To provide advice, education, training and development for patients, other Pharmacy staff and members of the Multidisciplinary team.
