The role will be integral in supporting the provision and delivery of acute and holistic care of frail older people in our Acute Hospital at Home frailty beds. You will also be working with Pathfinder Urgent Care to support admission avoidance and facilitate early discharge of patients within the first 72 hours of their acute episode.

You will identify suitable patients for the Acute Hospital at Home frailty beds across the Emergency Department, Medical Assessment Unit, Same Day Emergency Care Unit, and on the wards and will support the assessment, evaluation and management of these patients. You will be managing patients in both the acute setting and also in the community.

We encourage applications from Nursing and Allied Health Professionals who are willing to work generically across traditional professional boundaries to provide holistic best practice care for patients.

The Acute Hospital at Home team seeks to manage individuals living with frailty who have an acute medical condition that would usually necessitate an admission to an acute hospital bed. With the support provided by the Acute Hospital at Home team, however, the individual will be supported to remain in their own home receiving a high level of clinical care, diagnostic and therapeutic interventions to manage the acute episode.

As we move towards a new era of integrated care and working collaboratively across organisational boundaries, the need for dynamic, focused teamwork is vital and you will be an integral member of the still developing Acute Hospital at Home team. You will be key in continuing to build relationships across the acute and community settings and will be provided with a wealth of learning opportunities.

As you will be working in the community and the acute hospital, you will need to hold a valid drivers licence and have access to your own transport.

Working Pattern: 37.5 hours per week covering 8am 8pm and will involve some weekend working. For the right candidate we would consider part time hours.
