Job summary
Adults Portsmouth provide community, and some inpatient, nursing, therapy and specialist services to adults in Portsmouth. Our community responsive-based services include a range of:
*The Community Nursing Service is a team of healthcare professionals who include District and Community Nurses, Healthcare Assistants and Associate Practitioners. Together working closely with other professionals to provide services for those who are housebound and living within the Portsmouth area.
*Community Rehabilitation Service (CRS) is a multi-disciplinary team made from staff employed by Health and Adult Social Care for residents of the Portsmouth City area. The service aim is to help individuals remain at home and to avoid unnecessary admission to hospital, and to facilitate discharge from hospital by offering assessment of immediate needs.
*The Urgent Community Response Service is a community-based crisis response service for people aged 18 years and over, who live in Portsmouth. It is provided by a multi-skilled team to patients in their usual place of residence with an urgent care need. The service aims to prevent hospital admission and involves an assessment (within 2-hours of referral) and short-term intervention.
Main duties of the job
*In this role, you will have responsibility for providing the highest standards of patient care through on-going assessment, planning, implementation, and evaluation of programmes of care.
*Working within the Nursing and Midwifery guidelines to have responsibility for the management of a defined caseload
*To meet the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive, and evidence-based and which promotes independence
*Responsible for co-ordinating the work of the team, influencing clinical decisions and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice, underpinned by theory and expertise
*Supervise junior members of staff and/or students on placements
*To develop and maintain a culture of service improvement which promotes quality and safety and striving to do better
*This is in line with the requirements set out in the NMC code which require all registered nurses to:
Prioritise people; Practice effectively; Preserve safety; Promote professionalism and trust.
About us
We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.
Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.
The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.
Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.
