Job summary

An exciting opportunity has arisen to join our health visiting team as an associate practitioner (AP) in Solent NHS Trust. Working alongside the health Visiting and Specialist Health visiting SEND team for children with special educational needs and disabilities. If you are passionate about improving the health and wellbeing of children and ensuring they meet their full potential, then this role could be perfect for you.

You will work primarily with children from birth to 5 years and their families, alongside experienced Health Visitors (0-5) to deliver elements of the Healthy Child Programme (DH 2009).

Successful applicants will demonstrate excellent communication skills, be passionate about public health, show professional resilience and be creative and flexible in their approach.

To be successful in this rewarding and innovative role, we are looking for experienced early years practitioners.

Main duties of the job

Associate practitioner to work with the 0-5 service, supporting the children and families team with the healthy child programme and the high impact areas. Completing short- and medium-term interventions around behaviour and parenting.

To implement and evaluate packages of care at Universal, Targeted and Specialist levels of the Healthy Child Programme which is led by Health Visitors.

To work in partnership with other agencies and families and contribute to innovative ways of working with children and families.

About us

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.

Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.

The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.

Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

Qualifications

Essential

  • The Associate Practitioner will hold a qualification such as one of the following, or equivalent experience:
  • NVQ Level 4 in Health and Social Care or equivalent experience
  • BTEC National Diploma in Childrens Care, Learning and Development
  • CACHE Level 3 Diploma in Child Care & Education
  • City and Guilds Work with Parents NVQ level 4 qualification or ability to complete within 6 months of appointment.
  • Proven experience of working with parents and children

Experience

Essential

  • Experience of working with targeted families with children 0-5 and the delivery of targeted/specialist interventions.
  • Experience of working in a constantly changing environment
  • Able to work as part of a team
  • To work with children in a strengths-based way.

Desirable

  • Experience in a leadership/managerial role
  • Experience of leading/managing people in a Health or Social Care setting
