Job summary

A fantastic opportunity has arisen to expand our Integrated Community Childrens Nursing Service and appoint a highly skilled, enthusiastic and forward-thinking Registered Childrens Nurse!

You will provide specialist nursing contribution to the multi-disciplinary team in order to provide comprehensive assessment, treatment and management to Children and Young People who require nursing intervention. The role works across Portsmouth and the South East Hampshire area and with a variety of acute and long term conditions, complex needs, life-limiting and life-threatening illness, including Children and Young People requiring palliative and end of life care.

The iCCN service aims to provide a high quality nursing service which enables early, safe planned discharge from hospital and prevent unnecessary admissions providing a holistic approach to care within the community.

Our service is a link between Primary, Secondary and Tertiary care and encourage the development of partnership working and a seamless patient journey. This is a 7 day service and the successful potholder will be expected to work a range of days, weekends and the occasional on-call. With support, you will be expected to work autonomously as well as within a team environment, working closely with other members of the multidisciplinary team.

Main duties of the job

The Senior Community Children's Nurse is responsible for the ongoing assessment of care needs and the development, implementation and evaluation of programmes of specialised nursing care. Providing specialised skilled nursing care to children with a variety of nursing needs, including: long term conditions and complex needs, life limiting conditions, palliative care, acute short term nursing interventions as well as care for children following an urgent care pathway. The care will be provided in children's homes, schools, nurseries and across other community settings. Safeguarding is a fundamental part of the SiCCN role and requires a sound knowledge base to safely manage the caseload. The role will also involve provision of community children's nursing services working with partners in acute trusts and primary care networks as part of an integrated children's nursing service.

You will be fully supported in your role with a 4 week supernumerary induction to work alongside the team, gain competencies and shadow other healthcare professionals. We are proud to offer in house and external training opportunities such as central line and tracheostomy care to enhance your nursing skills.

Throughout your time with us you will be provided with regular clinical and safeguarding supervision and will be supported with continuing professional development and NMC revalidation.

About us

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.
