Job summary
This is an exciting opportunity to join our Vocational Rehabilitation Service on a fixed term contract providing 'return to work' education, advice, rehabilitation and support to patients with a Long Covid diagnosis.
This role would be best suited to an individual with experience working within health or social care settings, has experience working with people and has good communication and reasoning skills. The ideal candidate will be confident working virtually, using IT platforms for daily communications with both the clinical Team and patients both one to one and in groups. This post requires an NVQ level 3 or equivalent experience relevant to health and social care. Existing coaching skills and awareness of both the physical and mental health impacts of a long covid diagnosis would be desirable.
Solent Long Covid Service is available to Southampton and Portsmouth populations. Service delivery includes assessment, digital innovation application, and group work. The service is multidisciplinary crossing primary, secondary and community care. The service core clinical staffing of community physiotherapy, occupational therapy, nursing and non-clinical care co-ordinators with weekly multi service and profession MDT's for complex patient care in parallel or signposting. Care co-ordinators for each locality support patients navigate the complex pathways and support in the treatment pathway.
This post is fixed term until 31st March 2025. We will consider part time applicants.
Main duties of the job
As guided by the responsible clinician, prepare and deliver follow up appointment outcomes to identified patients.
Work independently within clearly defined protocols.
Co-ordinating multiple and complex pathways of onward referral spanning regional area.
Contacting local providers, community care provision to support patient care and journey.
Reviewing patient records to ensure timely diagnostics or other test reports are received and available for the responsible clinician to review.
Supporting the responsible clinician of the patient in onward referral, document processing, requests.
Excellent communication skills to relay important and clinical information.
Technological ability with a variety of systems to support digital/virtual group sessions.
Monitor app messages and ensure responsible clinician is sighted and aware of messages where appropriate and of clinical nature.
Work closely with the responsible clinicians, supporting journeys available to the patient and carers.
Acting as a single point of contact for patient to navigate the health care system.
Breaking down traditional barriers between health and care organisations.
About us
We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.
Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.
The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.
Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.
