Are you looking to be front and centre of developing a new and exciting community 'reablement' service in Southampton city?
We are looking for a senior occupational therapist who would like to join us on our exciting journey into reablement services, working alongside our adult social care colleagues to provide therapy intervention to patients to either aid safe hospital discharge or prevent hospital admission. The longer term aim being to support patients to return to their pre-morbid abilities and help reduce packages of care by providing therapy intervention, equipment and teaching to patients, families and carers.
This is a new service and we are looking for a motivated occupational therapist to join our team of occupational therapists, physiotherapists and urgent response care co-ordinators to help redesign, deliver and outcome the service. Under the supervision and support of the operations manager and therapy lead of STS/Reablement services.
Alongside this the therapy team will continue to deliver high quality rehabilitation services to patients on the short-term stays pathway in Southampton in local nursing homes with the aim for them to return home within a 5 week admission.
Main duties of the job
Work within the therapy team to provide therapy and rehabilitation to elderly patients (65yrs and over) with complex clinical needs who are discharged from hospital to a temporary nursing home bed for a 5-week admission to progress with the aim to return home, working alongside hospital discharge adult social care team..
Reablement
Work alongside urgent response social care team in patients own homes across Southampton for a period of 2-6 weeks dependent on patient need and goals. To provide assessment of patients within 24-48 hours of receipt of referral. To create rehabilitation/reablement programmes for the social care team to implement. Aim to improve patients independence, activities of daily living, provide equipment and aim to reduce care packages for individual patients.
Manage a caseload with supervision as required/be responsible for a defined area of service, determining daily occupational therapy and multidisciplinary rehabilitation intervention, and able to plan and implement comprehensive programmes of care for people who require intensive support / rehabilitation, guided by more senior clinicians where required.
Participate in and maintain own clinical professional development (CPD) by keeping abreast of new developments and incorporating into practice under guidance from supervisor and providing supervision and assessment rehabilitation assistants and students on placement.

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.
