Job summary
The Community Physiotherapy Service provides a service to adults aged 18 or over who require physiotherapy in their own homes because they are housebound or suffer with other problems relating to physical frailty which mean their physiotherapy needs are better met at home. We are looking for a Static Band 5 Physiotherapist to join our Community team!
As a Community Physiotherapist, it is essential that you have a full UK Driving Licence and access to a car.
We are looking for a bright, enthusiastic person with a background of NHS Physiotherapy who has a passion to support people to remain independent within their communities. The aim of our service is to promote patient's independence, reduce pain, reduce risk of harm and improve quality of life.
Our team has a range of staff from band 4 - 7 who provide direct patient assessment and treatment both in gymnasium settings in the form of Falls Prevention Classes and in the patient's own home. Previous rotational experience is desirable and you will be encouraged, and required, to develop skills and knowledge related to our client group to enable you to provide a high quality holistic care. We work with multiple external agencies so team working with other health and social care professionals is essential.

Main duties of the job
In our team care is highly patient centred and goal focussed. Our team is a responsive team that regularly is commended by its patients for its caring approach and short waiting times.
The service works closely with Community Nursing, GPs, Occupational Therapists, Social Workers and other members of the primary, intermediate and secondary care teams. Our service receives referrals from staff and patients. Referrals may come from hospital or community settings.
The post holder will be supported with a comprehensive induction and regular clinical supervision.
The post holder will work clinically across both our triage team and the direct patient facing element of our team. They successful applicant will see an exciting diversity of patients with a wide range of conditions and with a wide range of needs.
