Southampton Urgent Response Service are recruiting a Community Healthcare Support Worker!
You will work closely with a range of friendly professionals to provide clinical support across Southampton City. We provide care and rehabilitation programmes to clients who require immediate intensive care to enable them to stay in their home environment and reach an optimum level of self-care. This includes help with personal care, medication administration, meal preparation, and encouraging mobility/supporting patients with exercise programmes.
You will undertake tasks such as monitoring of vital signs, applying wound dressings, ECGs, bladder scanning and taking bloods. We'll provide you with training and support to achieve competencies in these areas. We are looking for people who are caring, hard-working, have effective communication skills, and are flexible. You will need to be able work over a 7 day period, working between the hours of 8:00 and 22:00. We will provide a full induction, regular support along with excellent ongoing training opportunities, so you can develop new skills and strengths.
We're looking for people with experience working within a health or social care environment at NVQ level 3 (or equivalent level of experience), whether you are already working at Band 3 level (or equivalent) or are an existing Band 2 looking for career progression this could be an ideal opportunity for you.
You will require access to a vehicle for travelling for which a mileage allowance is payable
Main duties of the job
All Community Support Workers will have a core basis of clinical skills and competencies. After appropriate training and development, to provide physical and psychological care to patients as part of a specified plan of clinical care. To record timely and accurate patient information.
Cleaning eyes and administering eye ointment/drops if required.
Oral hygiene and cleaning of dentures.
Skin assessment and recognition of signs of pressure damage to skin integrity.
Administration of skin emollients and other topical applications -- following competency achievement.
Nutritional Assessment, including MUST.
Mobility assessment and environmental factors -- Falls Prevention and seeking advice from registered staff if required e.g. unsafe environmental factors.
Assessment of Psychological & Spiritual Needs, Continence Re-assessments, Tissue Viability and Pressure Sore Prevention.
Wound Care on specified wounds following prescribed care plan and achievement of wound care, competency and application of compression bandaging.
Undertaking prescribed exercise programmes
Supporting with End of Life Care
Undertaking physical observations as required, blood pressure, temperature, pulse, respirations, blood sugars, urinalysis, weight and height and other therapeutic and nursing tasks identified in competency framework to support physical and psychological well-being.
