Job summary
This is an exciting opportunity for a motivated Physiotherapist to join a dynamic multidisciplinary community rehabilitation service in Southampton. The Community Independence Service delivers programmes of rehabilitation working in partnership with other social and healthcare providers.
The role will allow individuals to gain experience in provision of equipment and minor adaptations to support independence, falls assessment, education and prevention, complex community rehabilitation, goal focused rehab planning, providing advice on health and wellbeing and signposting to other community services. This will be supported by therapy staff, an advanced clinical practitioner and geriatricians.
The team promotes inter professional working which supports the single assessment process for patients. There is a robust supervision and appraisal frame within the team and opportunities for training and development. You will need effective interpersonal skills and to show skills to support a flexible and positive approach to problem solving. Main duties of the job
Responsible day to day for a designated case load of patients in the community, requiring physiotherapy intervention and undertake to organise and prioritise the required programmes of treatment, seeking support and advice with cases from more experienced colleagues when faced with matters outside of sphere of responsibility, knowledge or skills.
Assess patients understanding of treatment proposals and obtain informed consent to treatment and work within agreed guidelines for patients unable to give informed consent. Obtain consent to the sharing of information and document all consent issues in accordance with relevant Trust procedures.
Undertake specific physiotherapy assessments when indicated, using investigative skills for those with complex/rehabilitation/chronic needs e.g. patients with multi-pathology, terminal illness, severe disability, for example:
*Functional assessments
*Assessments of upper and lower limb motor and sensory function
*Assessment of gait & balance
*Postural and wheelchair seating assessments
*Assessment of mobility and related equipment needs.
*Assessment of gait and falls risks
*Assessment and management of respiratory disorders (e.g. airway management treatment methods including suctioning)
