Job summary
We are excited to be recruiting to the position of a 'Enhanced Mental Health Access Facilitator.' We are expanding our recently formed vibrant Portsmouth Mental Health Hub out into the community to provide enhanced provision to the GP Practices and their patients within a specific Primary Care Network located in our diverse inner city.
We are looking for candidates with an enthusiastic drive that have a sound knowledge of mental health to work in a creative and personalised way to provide advice, guidance and support in the management and treatment of primary care patients. The role will include sharing education and training to the GP practice staff and connecting to relevant support available in our city.
The Portsmouth Mental Health Hub was requested and designed by our community to support early, easy access to support and advice to anyone over the age of 16 who has a concern about their mental health. It is a progressive service which reaches into the community to ensure that anyone can access the right support at the right time. The service are able to make direct referrals into secondary mental health services as well as connect residents to community support.
Main duties of the job
The successful candidate will be required to work flexibly across a 5 day week. If you have great communication skills and the drive to make a difference, our service is the place you can build on your skills and make a difference to people's mental health journey.
