Job summary

An exciting opportunity has arisen for an enthusiastic and motivated Band 4 Senior Rehabilitation Assistant to join the Neuro Early Supported Discharge Team within Solent NHS Trust. We have 2 x full time posts available. A progressive Band 3-4 would also be considered depending on experience; this would be discussed at interview.

The Neuro Early Supported Discharge Team is a Multidisciplinary team providing specialist intensive rehabilitation to clients with Acute Stroke and other Neurological conditions on discharge from hospital or as community admission avoidance. The post holder will be working alongside a team of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Nursing, Clinical Neuropsychologist and Rehabilitation Assistants working the Southampton and West Hampshire areas.

Working Pattern: Monday -- Friday with the expectation for some Weekend working.

A full and valid UK driving licence and access to a vehicle for work purposes is required.

Main duties of the job

Work as part of multi-professional neuro team, in close collaboration with other relevant services/agencies e.g. regional neurology unit, acute stroke unit, community rehab teams, virtual wards, rapid response etc.

Carry out safe moving and handling practice, for moderately to severely dependent clients within a community or acute hospital / outpatient environment e.g. person's own home, on a daily basis, in accordance with planned programmes of care and Trust policies.

When within the community setting (e.g. within a client's home) work as a lone practitioner with telephone support.Assist clients with rehabilitation programmes e.g. following prescribed exercise programmes (individual and small groups), activities of daily living, mobility and transfer practice.

Analyse and evaluate client progress at every treatment session and report back to qualified staff accordingly.

Use comprehensive information technology skills to support administrative and service development tasks within the team.

To manage a rehabilitation caseload as delegated by Team Leader. Working without direct supervision, responsible for assessing patients, progressing and implementing programmes of care and reporting back to registered staff.

Ensure professional communication at all times using various modes of communication including verbal and written- using Information Technology appropriately as per protocols and guidance.

Actively participate in multidisciplinary Team meetings and family meetings

  • Foundation Degree / Diploma or NVQ3 with experience equivalent to a foundation degree level

Desirable

  • Neurological specific training e.g. equipment, gait re-education, handling, Neuro/Stroke Rehabilitation, LTC management, Palliative care, Postural management, Continence Care

Experience

  • Experience of regular assessment and treatment of patients presenting with problems requiring therapeutic intervention, some of a complex nature.
  • Recent NHS experience of treating people with a range of stroke / neurological needs e.g. continence, mobility, balance problems, cognitive deficits, dysphagia.
  • Relevant experience of working within a multidisciplinary team in either a hospital or community setting (some of which has been gained within a UK setting).
  • Working within a multi-professional team.
