As a Healthcare Support Worker, you will be responsible for maintaining the safety and wellbeing of our patients, deliver personal care whilst promoting privacy and dignity and support patients with their nutritional, skin, continence and mobility care. We need you to be flexible to work a variety of shifts, rota's are provided 6 weeks in advance with the possibility of a bank contract too.
We operate two level 3 rehabilitation units, a Level 2b Neurological Rehabilitation unit and a Neuropsychiatric unit which provide a range of specialist rehabilitation. As part of Solent's exciting Futures project we are moving all our community rehabilitation units onto a single site with a new purpose built state of the art rehabilitation centre of excellence.
Fanshawe Ward and Lower Brambles both provide step- up and step-down beds supporting acute admission avoidance and rehabilitation to help our patients strive towards their best outcome.

Main duties of the job
- You will work as a member of a multidisciplinary team alongside service users to promote independence and daily living skills, including personal hygiene, diet and fluids, encouraging them to engage in activities as identified within their care plan.
- Act as a role model for team members, supporting new or less experienced colleagues.
- This role will assist and support qualified staff and carry out assigned tasks as directed, without supervision.
- This role will demonstrate excellent communication skills, engaging, encouraging or negotiating with clients with complex, at times challenging needs to ensure they are provided with the most appropriate care and support.
- Actively promote recovery-focussed care with patients, in conjunction with other health professionals, including achievement of set goals.
- Support patients with their rehabilitation and recovery, respecting them as individuals, supporting them and understanding their right to express their needs.
- Be the main provider of personal care for patients where required.
- Accurately assess, describe and record observations in patients' mental and physical state using appropriate assessment tools, escalating concerns as appropriate.
- Undertake clinical and environmental risk assessments and document key risk areas, seeking guidance from senior staff when dealing with cases indicating a high level of risk.
- Raise incidents appropriately in an accurate, objective and concise manner.
- Undertake escort duties to other hospitals as required.
