Job summary

There is an exciting opportunity for a motivated and passionate individual to join our innovative mental health AHP service.

The post holder will join the older persons specialism within the AHP service and provide support to older adults with complex mental health needs across inpatient and community settings. The post holder will work closely with colleagues to deliver evidence based assessments, interventions and groups as well as provide specialist support and advice to the wider service and MDT.

There is professional and clinical supervision available from a supportive and multi-disciplinary leadership team. Our team emphasises innovation, development and evidence base and therefore encourage personal development and sharing of skills through regular supervision and appraisal.

Main duties of the job

• The post holder is responsible for the provision of a high-quality Occupational Therapy (OT) service, ensuring that the service delivered meets Trust and professional standards and develops in line with Government, Clinical, Professional, and local guidelines.
• Delivering occupational focused assessment, formulations, and interventions for a caseload of patients with complex needs across multiple settings.
• To deputise for AHP Team Leads and provide cover for the other team members as necessary.
• To be responsible for supporting, developing, and evaluating an evidence-based therapeutic group programme.
• Act as a Lead Professional for a limited number of patients with complex needs ensuring the full implementation of the Care Programme Approach Policy.
• Act as a link professional within a specific team to be appoint of contact for referrals, provide clinical and professional advice and represent the AHP service during multi-disciplinary meetings.
• To be a key resource in facilitating the patients journey through mental health clinical pathways within acute, primary, and secondary care. This will involve supporting patients in their homes and communities to prevent an avoidable hospital admission, and to work with patients in hospital to ensure safe, effective, efficient, and recovery focused discharge.
