Job summary

We are looking to appoint a highly skilled, enthusiastic, and forward-thinking Senior Community Children's Nurse to our Community Children's Nursing (CCN) team.

Our established, passionate, and dedicated CCN team work collaboratively with children, young people, families, and the wider multi-disciplinary team to ensure care is provided as close to home as possible. We provide high quality and holistic, family centred care to children and young people (CYP) with a nursing need across the Southampton and west Hampshire area including those with.

We are looking to recruit a Registered Children's Nurse who has undertaken a Specialist Practitioner qualification in CCN with experience of caseload management. A Senior CCN will have the opportunity to shape and lead our responsive service as it continually adapts to the ever-changing needs of the families. Having a key role in leading care plans, training, and multi-disciplinary liaison for CYP with a range of needs within their home, school, and wider community.

This opportunity will provide experience of working with CYP and families in the community as well as the chance to transfer a wide range of clinical skills. This role will also enable you to further develop your leadership and management skills.

Main duties of the job

*Manage a caseload. Provide specialised, high quality skilled nursing care, considering the physical, psychological, spiritual and social needs of the child with nursing care needs. This will include those with complex health care needs, palliative care needs, life-limiting illnesses and children dependant on technology (and those requiring short term support). This requires developing, implementing and evaluating individualised programmes of care.

*Participate in the administration of medicines including intravenous therapy and cytotoxic drugs, in partnership with pharmacy colleagues. Be aware of current drugs/therapies in the treatment of pain and other symptoms. This requires specialist knowledge of children's palliative care issues and symptom management.

*Maintain regular liaison with local paediatric unit, paediatric emergency department, primary health care team and other agencies to ensure good communication and continuity of care for the child. Working collaboratively with all professionals to enable children/young people to be cared for at home whenever possible. The information received is often of a complex and sensitive nature requiring sensitivity, empathy and reassurance.

*Participate in the development and delivery of training programmes for all health and social care colleagues. Utilising a one to one or group approach as required.

*Deliver as part of a rota and on call, care for children at end of life
