If the answer is yes we have an exciting opportunity for you. The Solent Care Home Team is looking to recruit a Band 7 Matron. To support the delivery of essential nursing care, assessment and treatment throughout our Portsmouth Residential and Nursing homes. You will use all your skills and knowledge within this diverse post to support patient experience, wellbeing, recovery, rehabilitation and End Of Life care.

As well as playing an active role in the face to face element of patient experience, you will also be using your leadership skills to support education and development within both health and social care areas. The team offers a unique opportunity to work with a wider multidisciplinary team including Adult Social Care, Private Care Providers, General Practitioners and wider teams.

The teams hours 8:30-4:30 on a 5 day rota. Weekend working will be required as part of the rota with the team.

Main duties of the job

To provide expert clinical case management and support for patients with complex care needs living in Nursing & Residential Homes, who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission.

Work with a high degree of autonomy and professional accountability.

Diagnose, prescribe, and implement a range of core interventions with an emphasis on avoiding hospital admission or an unnecessary longer length of hospitalisation.

Provide specialist, expert clinical knowledge and leadership whilst directly/indirectly supervising other team members.

Use advanced skills and expert knowledge and perform comprehensive and systematic physical examination.

Planning care interventions for people with multiple and complex long-term conditions, working with the multi-professional team. You will work collaboratively with Care home and Nursing home staff and Primary Care Networks in managing the health needs for the designated patient group.

You will participate in the development, implementation and evaluation of the delivery of health care services and be part of the development of health services and inter-professional working.

Working collaboratively with Adult Social Care supporting home closure.

To lead, facilitate and provide clinical advice and expertise to the Multidisciplinary team meeting programme within the Care homes.

To provide timely advice and planning support to Care Homes that require intensive support following CQC inspection.
